We offer support, connection, and guidance for stroke survivors and caregivers as they adjust to life after the hospital.

All programs are free and open to the community.

Connect with someone who has been through stroke recovery and understands what you’re going through. Peer mentors offer encouragement, guidance, and real-life insight.
